Disclosure Application Forms are now available from:
Provincial Youth and Children’s Office, 25 Lynwood Avenue, Slough, Berks SL3 7BJ

Both Standard and Enhanced Disclosures are free for voluntary (unpaid) workers.
Standard Disclosures for paid workers will cost £26 and Enhanced Disclosures will cost £44.
