Applying for a Disclosure



Disclosure Application Forms are now available from:

Both Standard and Enhanced Disclosures are free for voluntary (unpaid) workers.
From 1st April 2005 Standard Disclosures for paid workers will cost £29 and Enhanced Disclosures will cost £34.


Please follow the guidance notes below when completing your Disclosure application

It is essential that you complete the form in BLACK ink. The CRB will reject applications completed in any other colour.

Do not use correction fluid anywhere on the form. The CRB will reject any such applications. If you make an error, cross through it once and then write the correct information next to it or in a blank space nearby.

Section A
Please complete all the fields in Section A.

When completing line 3 it is essential that you include any middle names.
Section B
In Line 13 write the title of your role in church, for example ‘voluntary junior church leader’ or “toddler group assistant”. Please make the nature of the role as clear as possible, including “voluntary” or “volunteer” to ensure that you will not be charged for your Disclosure. If, from the nature of the title, it is not clear that the role relates to children, young people or vulnerable adults, then please provide more information, either on the CAS form or on an additional sheet.

In Line 14 write the name of the organisation you will be working for, for example, ‘Swindon Moravian Church’.
If you are a minister applying for a Disclosure please write ‘The Moravian Church’ in this section.

In lines 15-19, please write the address of the church or organisation where you will be based.
If you are a minister, please give the address of Moravian Church House, 5 Muswell Hill, London N10 3TJ.
Section C
Lines 20 – 26/27. If you have ever used a different forename or surname from the one you use now, you must complete Lines 21 to 26/27.

Lines 21 and 22 Female applicants must complete these lines if, in Line 1 of Section A, they have given their title as Mrs, Ms or another title (e.g. Dr, Rev.), even if they have never been known by a different surname. This is so that local police forces do not query the fact that you may not have disclosed a different surname, which would delay your application.

Lines 28 – 35 Please complete all of these lines.
Section D
You must provide a FULL five-year address history without any gaps or overlaps in the date. For example, if you are making your application on 1st January 2005, your address history must go back to at least December 2000. A separate sheet may be used if necessary.

Please ensure that your name and application reference number (at the top of the front page of the CRB form on the right) is written on any additional sheets used. Please follow the date format required by the CRB (MMYYYY).

Sections E, F and G should NOT be completed. Details of payments will be entered by the CAS.

Please note that if your role is a paid one then you will need to send a cheque, made payable to ‘Methodist Church Fund’ with your application. The current fee for a Standard Disclosure is £28 and for an Enhanced Disclosure it is £33. Fees for ministers can be reclaimed from the Provincial Board.
Section H
Lines 66, 68 and 70 must be completed.
Line 69: consenting to the CRB using commercial data sources to verify your identity speeds-up the application process. If you do not wish to give your consent, additional identifying documents will be required for Section X – please refer to the Documentary Evidence Sheet.

Once the above Sections have been completed, you will need to take the form to a Moravian Minister or Church Secretary/Treasure whose name is listed in the back of the Daily Texts, together with your identity documents.

If you are applying as a Minister, please arrange for a fellow Minister or Board Member to check your documents.

Please refer to the Documentary Evidence Sheet for details of the documents that may be used and the number required.

Only original documents are acceptable. At least one document must show your address which must be your current address. At least one document must show your date of birth.

Please note that, apart from a Birth or Marriage certificate, Group 2 documents must be either less than 3 months’ old or have been issued within the 12 months preceding the application – please refer to the CRB guidance notes (page 1) for details.
Section X
The person verifying your identity will check your documents and record the necessary information. Some of the documents you present to your verifier may not be listed in Section X, in which case your verifier must record them on the enclosed Documentary Evidence Sheet (CAS2). The verifier must print his/her name in Line 16 of Section X. The verifier should only place a cross in line 15 if he/she has seen two group two documents (not listed in Section X) showing your current address.
SECTIONS Y AND Z SHOULD NOT BE COMPLETED



THE CAS FORM
This must be completed, signed and dated by your verifier before being signed by you.



SENDING YOUR APPLICATION
When you have completed the Disclosure application form, please send it with the CAS form and the Documentary Evidence Sheet (if used) to:

DO NOT SEND THE APPLICATION FORM TO THE CRIMINAL RECORDS BUREAU – IT WILL DELAY THE PROCESS INDEFINITELY!!
